What You'll Do:
• Meet with patient/clients to establish medical histories, exercise histories and exercise goals.
• Develop, implement, and coordinate exercise and wellness programs for patients/clients under supervision of a physical therapist or physician.

DETAILED JOB DESCRIPTION
Operate equipment for performing electrocardiograms, event monitor and Holter monitor application, scanning, and analysis, and exercise stress testing. Identify unusual or dangerous arrhythmias, ischemic EKG changes, symptomatic patients, and positive stress test results and appropriately notify the physician. Obtain the patient’s vital signs and brief medical history. Perform and document brief physical assessment, i.e. heart and lung auscultation, edema evaluation, etc. As part of pre-test evaluation, screen patients, charts, and physician request forms to ensure proper test and protocol, utilizing established criteria to determine the level of physician supervision required to safely perform each test. Assist in training staff, medical students, residents, fellows, and attending physicians in the correct operation and use of technical equipment, testing protocols, and stress test reporting forms.
Monitor inventory and maintain an adequate supply of materials for stress lab, crash cart, etc. Manage the stress test reporting system, both during testing and post-test calculations. Act as staff content expert for exercise and stress testing education for the fellows, residents, and medical students. Support clinical research efforts and integrate exercise science theories into clinical practice. Assist in the development and management of the aerobic and strength-training programs for cardiac and pulmonary rehabilitation programs. Assist in monitoring cardiac and pulmonary rehab Phase II patients, emphasizing patient education, risk factor and lifestyle modification, and development of aerobic and strength training programs. Utilize telemetry monitoring of EKG and heart rate, RPE (Rating of Perceived Exertion), and dyspnea scales, and target heart rate ranges to maximize the efficacy of workouts.
Prepare, schedule, and teach cardiac and pulmonary rehabilitation education classes to improve patient awareness of exercise, muscular and cardiovascular adaptations, stress management, coronary artery disease, risk factor modification, smoking cessation, and diabetes control to improve overall knowledge and health status. Evaluate Phase I inpatients, including reviewing medical files, vitals, EKGs, and stress tests and record vitals and telemetry results to develop an initial workout program. Participate in multidisciplinary rounds with physicians. Establish and maintain a knowledge base of testing modalities both for stress lab and for measurable pre- and post-rehabilitation outcomes such as 6-min walk, % body fat, etc. Complete chart documentation; verify patient account charges, review rehab patient attendance and discharges to ensure accurate information is recorded. Stay current with the latest developments, advancements, and trends in exercise stress testing and cardiac and pulmonary rehabilitation. Identify ways to improve work processes, maximize quality and efficiency, and improve customer satisfaction.
Evaluate the exercise programs and progression of Phase II and Phase III outpatients in the gym.
Review charts and conduct vitals and stress tests to determine an exercise treatment plan. Prescribe individualized exercise program. Maintain knowledge of applicable rules, regulations, policies, laws, and guidelines that impact the stress lab and the cardiac and pulmonary rehabilitation area. Develop effective internal controls that promote adherence to applicable state/federal laws and the program requirements of accreditation agencies and federal, state, and private health plans. Seek advice and guidance as necessary to ensure proper understanding. Communicate effectively with patients, family, staff, and management. Maximize customer satisfaction by providing patient-oriented guest services. Demonstrate a leadership role in monitoring quality and identifying and implementing improvements in efficiency, service, and patient care. Develop and manage the stress-testing lab in a cost-efficient manner. May complete unit/department-specific duties as outlined in department documents.

Title: Exercise Physiologist II - Vassar Brothers Cardiac Rehab - Poughkeepsie, NY - Full time/40 hours per week, 8 hour shifts, 8:00am - 4:30pm.
Education/Experience required: Master’s degree in exercise physiology. Minimum 5 years experience in cardiac rehabilitation. Proficient in EKG telemetry reading. License Required: BLS/ACLS
Summary: Assists patients with their exercise programs to include the performance of health risk assessments, exercise testing, program guidelines and education with patients. Responsible for patient preparation, testing, data analysis, and report generation for cardiopulmonary exercise testing (CPET) program.
Responsibilities:
1. Develops, coordinates and implements exercise related programs specific to each patients needs, following American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) Guidelines. Creates a safe environment for learning that promotes a patient's understanding of the disease process and maintaining a healthy lifestyle.
2. Monitors, interprets, and documents EKG rhythms during cardiac rehab. Reports atypical signs and symptoms as well as significant EKG changes to cardiologist.
3. Educates and instructs patients during exercise class to provide educational materials and ensuring safe, appropriate use of equipment and execution of activities.
4. Conducts cardiopulmonary exercise testing (CPET), as well as 6-minute walk tests accurately collecting, analyzing, and recording data during exercise testing.
5. Manages CPET program by preforming CPET testing. Prepares and finalizes reports for physicians and other healthcare professional. In collaboration with physician, analyzes CPET data to identify abnormalities and/or trends.
6. Monitors, interprets, and documents heart rate and O2 saturations during pulmonary rehab exercise. Reports untoward signs and symptoms to pulmonologist. Instructs and leads weight and strength training class for pulmonary rehab.
7. Maintains and Models Nuvance Health Values.
8. Demonstrates regular, reliable and predictable attendance.
9. Performs other duties as required.

The Exercise Physiologist supports the health and wellness of patients by assessing fitness levels, prescribing safe and effective exercise programs, and providing education to promote long-term health improvements. Key Responsibilities:
· Provide comprehensive oversight as the Facility Lead
· Act as a liaison for facility maintenance with contractors and ensure effective oversight of the budget for new equipment.
· Monitor vital signs, including heart rate, blood pressure, oxygen saturation, symptoms, and blood glucose levels, ensuring accurate documentation within Epic, Duke's electronic medical records system.
· Formulate personalized exercise prescriptions tailored to clients with diverse health histories and varied goals.
· Offer individualized personal training services to a diverse client base, accommodating a range of health histories and objectives.
· Coordinate and lead small group exercise programming with precision and attention to individual needs.
· Maintain regular communication using fitness center technologies such as club Management software, Epic, Smartsheet, and Microsoft Teams.
· Actively contribute to internal marketing efforts by verbally promoting and cross-selling programs and services.
Work Performed:
Ensure a secure exercise environment for clients, monitoring vital signs and collaboratively communicating care plans with the complete healthcare team. Spearhead the planning, coordination, and implementation of exercise and related programs tailored to diverse special populations, clinical programs, and healthy demographics. Undertake the responsibility of creating personalized exercise prescriptions, setting goals, and orchestrating exercise progression for patients at varying risk levels—low, medium, and high. Conduct comprehensive fitness assessments for clients spanning a spectrum of health abilities. Provide personalized training and small group sessions, catering to both clinical and healthy populations. Take charge of facility management to ensure daily maintenance of the pool and spa in compliance with state guidelines, including thorough documentation and staff training. Coordinate schedules for routine upkeep and maintenance in collaboration with on-site Engineering and Operations teams. Serve as a liaison between staff and client feedback for fitness equipment repairs, managing communication with our equipment maintenance company. Lead budget planning for the regular procurement of new equipment to meet the evolving needs of the facility. Function as an integral team player, contributing to optimal client care and fostering mutual support within the team.
Required Qualifications at this Level
Education
Master's degree in Exercise Physiology or Exercise Science. Bachelor's degree plus ACSM RCEP credential accepted
Experience
Level I: Minimum 1 yrs applicable experience, prefer 2
Level II: Minimum 3 yrs applicable experience (leadership experience preferred).

PREFERRED EXPERIENCE AS TYPICALLY ACQUIRED IN:
• 2 years experience in an outpatient care setting, performing exercise procedures on patients.
• 2 years experience in applying principles of exercise physiology for fitness acquisition, utilizing effective instructional educational techniques, clinical protocol development and interdisciplinary team processes.
SKILLS AND KNOWLEDGE:
• Knowledge of anatomy, physiology and pathophysiology, chemistry, and physics.
• In-depth knowledge of exercise testing, exercise equipment and quality control.
• Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Ability to perform multiple tasks and prioritize independently with follow through in a timely manner.
• Ability to design, implement, and supervise individualized exercise prescriptions for people with chronic disease and disabling conditions.

Provides all aspects of phases I – III cardiac rehabilitation. Under general direction, develops, implements and evaluates exercise prescriptions and exercise sessions for all phases of cardio/pulmonary rehab as well as other chronic illnesses. Creates educational materials and provides education both cardio/pulmonary patients and other patients referred to the cardiac rehab program. Plans and participates in special patient events, writing/research endeavors, professional conferences, the student internship program, specialized programmatic service lines (ie, Cardiovascular Performance Clinic, cardiac rehab referral intake, cardiac rehabilitation marketing, etc.), and/or departmental improvement initiatives related to the department. Extremely knowledgeable of cardiac risk factors. Provides all aspects of care management to the Phase II patients in an effective, timely, inspirational manner.
Supervises cardiovascular stress tests for individuals with and without chronic disease in order to obtain reliable and valid data.
Provides other services such as Enhanced External Counterpulsation (EECP) and holter and event monitor placement.
STANDARD QUALIFICATIONS

• Bachelor’s degree in exercise science or related field. Master’s degree preferred.

• Current BLS Certification- required; Current Advanced Cardiac Life Support- preferred (may be required at specific sites); ACSM Certified Clinical Exercise Physiologist or AACVPR Certified Cardiac Rehabilitation Professional within one year of employment.
