Social Worker

Empowered to Thrive is seeking an experienced and self-driven Social Worker to join our growing private practice on a locum basis with strong potential to transition into a full-time caseload.

This role is ideal for a clinician who values flexibility, autonomy, and income growth, and is comfortable starting with a smaller caseload that will build over time.

You will be based at a rehabilitation facility at Tokai Estate, supporting patients with complex medical needs (neurological, orthopedic, surgical, and respite care).

Your role will focus on:

• Psychosocial assessment and discharge planning
• Supporting patients and families through counseling and mediation
• Coordinating care and connecting to appropriate services
• Reducing barriers to recovery and engagement

• Flexible Structure: You are required on-site daily, but may leave once your patient work is complete
• Income Growth Model: Start with a smaller caseload and grow into a full-time, higher-earning position as referrals increase
• Autonomy with Support: Work independently while being part of a respected interdisciplinary team
• Administrative Support: We handle authorizations and billing so you can focus on clinical work

This role is best suited to a social worker who:

• Is comfortable with variable income initially in exchange for long-term growth
• Is self-motivated and proactive in building and managing a caseload
• Values flexibility over rigid structure
• Is looking to grow within a private practice setting, not just fill hours

• Current SACSSP registration
• Professional indemnity insurance
• Minimum of 3 years social work experience
• Experience in hospital social work and case management
• Training and experience in palliative care (essential)
• Strong report writing and time management skills
• Ability to work collaboratively within a multidisciplinary team
• Willingness to travel for outpatient consultations

Advantageous: Experience in geriatrics, oncology and curatorship

• 60% commission on services rendered
• Earnings increase as your caseload grows
