Position Details Title: MDS Coordinator (LVN) Schedule: Part-Time Pay Range: $35.00 - $42.00 per hour , depending on experience What You'll Do Coordinate and complete accurate, timely MDS assessments in compliance with state and federal regulations Ensure care plans reflect resident needs, clinical status, and regulatory requirements Collaborate with interdisciplinary team members to support quality outcomes and reimbursement accuracy Monitor documentation for completeness and compliance Support survey readiness and quality improvement initiatives Serve as a clinical resource related to MDS processes and best practices What We're Looking For Current, active LVN license in good standing Prior experience with MDS coordination in a skilled nursing or long-term care setting Strong knowledge of Medicare, Medicaid, and RAI processes Excellent organizational skills and attention to detail Ability to work independently while collaborating with a multidisciplinary team Strong communication skills and a positive, solutions-focused mindset What We Offer Competitive hourly pay based on experience Supportive leadership and team-oriented culture Flexible part-time schedule An opportunity to make a meaningful difference in resident care and outcomes Equal Employment Opportunity StatementWillow Springs Healthcare Center is an Equal Opportunity Employer.
