Overview:

Total Care Connections is seeking a highly motivated Intake Coordinator to support our growing operations in Arizona and Colorado.

At Total Care Connections, our nurse-led care team provides families throughout Arizona and Colorado with comprehensive in-home care services ranging from companion care to private duty nursing.

Job Summary:

The Intake Coordinator plays a critical role in supporting new client referrals, conducting intake coordination, building individualized client profiles, monitoring all new inquires, and ensuring a smooth transition into services. This position works closely with referral partners, clients, families, caregivers, schedulers, and operations teams to deliver an exceptional client experience.

Success in this role requires professionalism, responsiveness, strong communication skills, attention to detail, and the ability to manage multiple priorities in a fast-paced environment.

Duties & Responsibilities:

• Communicate with clients and families to identify care needs and service expectations.

• Track every inbound direct referral for weekly reporting

• Follow up with new referrals in a timely manner

• Deliver exceptional customer service with professionalism, empathy, and urgency.

• Maintain accurate and timely documentation within company systems.

• Partner closely with Scheduling, Recruiting, Care Coordination, and Leadership teams.

• Participate in team meetings and contribute to growth-focused initiatives.

Skills & Qualifications:

• Minimum of one year of experience in healthcare, home care, customer service, intake coordination, scheduling, or related field preferred.

• Excellent verbal and written English communication skills.

• Strong organizational skills with high attention to detail.

• Ability to multitask and manage priorities in a fast-paced environment.

• Professional phone presence and customer service mindset.

• Proficiency with computers, CRM systems, Microsoft Office, and email communication.

• Reliable high-speed internet connection and dedicated workspace required.
