Empowered to Thrive, a dynamic private practice based at Faircape Health Tokai Estate, is seeking a skilled Locum/Part-Time Social Worker to join our team. This is a unique opportunity for an experienced professional who is passionate about holistic, patient-centered care and values the flexibility to shape their own schedule.

At Empowered to Thrive, we support individuals navigating complex medical recoveries- including Neurological, Orthopedic, Surgical, and Respite care-by offering critical psychosocial services that promote well-being and safe discharge planning. Our social workers play a vital role in supporting patients and families through counseling, mediation, case coordination, and stress-reducing interventions to ensure optimal engagement in the rehabilitation journey.

Responsibilities:

  • Assess Care needs
  • Assist families to implement recommended care to ensure safe discharges are in place.
  • Connect patients to relevant resources such as care agencies, frail cares, day clinics, palliative care resources etc.
  • Make referrals to appropriate service providers such as psychologists, Department of Social Development etc.
  • Provide short term counseling
  • Provide mediation to patients and families
  • Provide practical assistance to patients to reduce stressors in order to participate in their rehabilitation.
What We're Looking For
  • SACSSP Registration (current and valid)
  • Professional Indemnity Insurance
  • Training and experience in Palliative Care (essential)
  • Additional experience in Oncology, Curatorship, or Geriatric/Frail Care (advantageous)
  • Proven ability in Hospital Social Work and Case Management
  • Strong report writing and time management skills
  • Comfortable working flexible hours within normal business times
  • Willingness to travel for outpatient consultations
  • Ability to collaborate effectively with an interdisciplinary care team
