Community Assessment & Support Officer - Huddersfield Royal Infirmary
Job Description
The post holder will be based and line managed by the Huddersfield Royal Infirmary social services team although there are 2 other members of staff who are based at Dewsbury District Hospital and between the recovery bed reviews are shared between the 3 of you.
The team work with adults over the age of 18 who have physical, sensory and mental health needs and who are in hospital. On occasions it is identified that some people require a longer period of time to recover before their final discharge destination from hospital is confirmed. Those individuals are then discharged from hospital in to a recovery bed to give time for further assessment.
This role requires the post holder to carry out person centred assessments of the individuals care and support needs to ensure a safe and effective discharge from the recovery bed. You will be expected to work with the individuals and their families/representative to identify their ongoing care and support needs.
We are looking for individuals who are well organised so that they can plan their work accordingly to ensure they meet the deadlines for reviews/assessments to support the wider health and social care footprint.
The postholder will be expected to work with the multi disciplinary team who work within the recovery hub base.
We are committed to providing our Community Assessment and Support Officer (CASO) to further develop their skills and knowledge through regular supervision, regular work load management, training and shadowing of other colleagues/professionals. We have an open door policy if you require to speak to managers for advice.
There are occasions when due to hospital pressures you may have to support the acute hospital social services team to carry out assessments and facilitate safe discharges.
There is an expectation that you when carry out assessments you meet face to face with individuals in the recovery beds however you can also work in the office where the HRI hospital team are based.
This post is working Mondays to Fridays only and there are opportunities to work from home when appropriate.

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all of most of the time.
It is essential that you hold a full and valid driving licence and it is expected that you will either use a council vehicle or your own care. (Exceptions may be made for disabled applicants).
